VIM Team Registration 2008 Survey

If you are leading a United Methodist Volunteers in Mission (UMVIM) team sponsored by the Office of Creative Ministries (OCM) please have your team members fill out, sign, have witnessed and notarize (where applicable) the following forms. Each form may be downloaded by clicking on the link below. You can request that forms be mailed to you by contacting johnson@umocm.com.

Attention Team leaders: Check all forms for signatures, notarizations and witnesses needed. Send forms and all fees 45 days before departure date. Late arrival may cause delays in processing.

General Forms

1. Liability Form for USA teams-sign and notarize and mail original to OCM.

2. General Board of Global Ministries Database Form-mail original to OCM.

3. Team Roster-send members names, address, email, phone and other contact information to OCM.

4. General Board of Global Ministries Insurance Form-sign and witness. Mail along with check, 45 days before departure, to GBGM address located on this form (cost = 75¢ x # members x # days). OCM has insurance that can be implemented with much shorter notice. The cost is $1.25/person/day.

5. Covenant Agreement-sign and give to team leader.

6. Medical Information/Consent-sign, notarize and give to team leader.

7. Emergency Contact Information-fill out and give to team leader.

8. Team Leader Evaluation-filled out by team leader following the mission and send to OCM.

9. Team Member Evaluation-filled out by each team

member following the mission and send to OCM.

*Extra International Forms

1. Death Notification/Consent-sign, notarize and give to

team leader.

*Extra Forms for Youth

1. 1. Parent /Guardian Consent-for USA (youth must have a copy as well)

2. Parent/Guardian Consent (International teams)-Both parents must sign and notarize. Be prepared to show it at border

crossings. The youth should have a copy as well. In an effort to prevent international child abduction, many governments have initiated additional screening procedures at entry/exit points. These often Include requiring documentary evidence of relationship and notarized written consent for the child's travel from the absent parent(s) or legal guardian. Having such documentation on hand, may facilitate entry/departure.
